Covid Return to PT Policy
ENCHANTED LAKE PHYSICAL THERAPY DAILY WELLNESS & RETURN TO PHYSICAL THERAPY POLICY
Before each physical therapy session, all patients must sanitize their hands, answer COVID-19 screening questions and receive a temperature check. We require a well-fitted mask to be worn at all times, while in the clinic.
Please report any illnesses, COVID-19 exposures/positive tests, and out-of-state trips to Enchanted Lake Physical Therapy by calling (808) 262-2292. Please send all test results to savannahsai.elpt(at)gmail(dot)com or chablis.elpt(at)gmail(dot)com. Enchanted Lake Physical Therapy accepts COVID-19 test results from medical testing facilities or state-approved testing sites. Home kit test results will not be accepted for clearance to return to physical therapy.
Anyone with COVID-19 symptoms, even if mild, should stay home from physical therapy. Please only come to physical therapy if you are symptom-free. A COVID-19 test is recommended if you are experiencing any symptoms.
